Sports Physicals

$25Brownsburg Athletics and IU Health are offering low cost pre-participation physical examinations for 2014-2015 sports participation at a rate of $25.00 per student, payable online or by cash.
No walk-ins accepted; participants must pre-pay when registering for an arrival time. Register online here or in person at the high school athletic office between 9 a.m.-3 p.m. on school days. These exams are available to all middle and high school athletes.
Students should print and complete this IHSAA physical form and bring it to their appointments.
